    Incredible and Helpful!

    Formation and Matt were so helpful and made every single person at our wedding feel safe and able to enjoy our big day! 10/10 would recommend using Formation for your event. Their vaccination and PCR test upload program is so easy and efficient. In the uncertainty of today’s world a must for wedding safety and Matt is so personal and flexible!!

    Comfortable during Covid

    Formation Health was great to work with! They showed up half an hour before I asked them to so they could get set up and be ready before wedding attendees started arriving. They were super patient with the guests through explaining the process until calling out their name to let them know of their test results. We received a lot of feedback from guests that they felt so much more comfortable knowing that the people they were going to spend the next 4-6 hours with all tested negative for the rapid tests. Because we decided to incorporate the rapid tests, about 10 guests decided to come to both the outside ceremony and the indoor reception. Thank you so much Formation Health making our guests feel safe so they could be there to share our special day with us!

    Saved My Wedding!

    Offering rapid testing at the door saved our wedding from cancellation. When I emailed them in a panic, they were calm and positive and helped me with clear next steps to get up and running. They even did a video chat with me to preview the wedding venue to confirm where an ideal pop-up testing location would be. Formation's organization and communication was excellent and leading up to and day-of the wedding. Everyone kept telling me how safe they felt at the wedding and how positive their experience was with the testing. I have now been to two weddings where Formation did the testing (one being my own) and both were tremendous successes. Working with Formation is a really great way to help your guests feel comfortable and cared for.

    Formation Health Helped Us Pull Off a Pandemic Wedding

    Formation Health proved to be a true lifesaver for our wedding. We decided the week before the event to add an extra layer of precaution and administer rapid tests to all of our guests. Matt calmly and clearly guided us through the process on the Monday before the wedding and then worked with our venue to make sure everything was up and running in an organized fashion. Ours was a weekend wedding affair with guests arriving at different times, and Formation proved incredibly flexible and adept at handling the logistics around this.

    Many of our guests commented to us how impressed they were with the on-site team. Everything was organized and efficient. The test itself was minimally intrusive for guests of all ages. The on-site staff were even kind enough to stick around later than planned because one family member was running late. We heard from many attendees that Formation Health and the offering of rapid tests allowed them to more fully enjoy the wedding. Kudos to Matt for such a smooth-running operation. We felt working with Formation was definitely the right choice and helped us pull off a successful pandemic-era wedding.

    Seamless COVID-19 Testing

    Formation Health was a breeze to work with. We decided to test at our August 2021 wedding two weeks before our event (stressful!) and even with everything being so last minute, Matt and his team were so understanding and accommodating. They gave us tons of options, helped us make a decision about who/how/when to test, and everything day-of was as smooth as can be. We are SO happy we decided to test (and happy we choose Formation!) - our guests kept saying how grateful they were and it made everything feel so much more comfortable.

    Amazing experience with formation health!!!

    Formation Health was absolutely FANTASTIC with our wedding this past June. They were extremely professional, communicated with us brilliantly, and did an amazing job on our wedding day. Would recommend them to any and all looking for covid testing for a small or large event!

    Helped Our Wedding Feel Safe

    Amazing customer service!

    We felt like we the #1 priority at every step. Matt and his team made sure that the forms were taken care of and the testing process smooth.



    Our guests had lovely experiences with the friendly techs even as they were getting swabbed!

    When guests showed up late, they stuck around to test them and make sure that our wedding was safe. Impressive flexibility that I wasn't sure we could count on.

    Would give them 6 stars if I could!

    Highly professional, highly recommend!

    When planning a wedding during covid, it was very important to us to make sure everyone had proof of vaccination or showed a negative covid test. Formation Health did an incredible, seamless job of handling all aspects of this for us. They are extremely professional and I would highly recommend Matthew and his team.

    Peace of mind

    My sons wedding was in January 2021, and we were very nervous about making sure everyone would be safe. Matt and his team from Formation Health were fabulous. All we did was give them a guest list and from there they took care of everything. I never once had to check in to make sure all was ok. The entire testing process went smoothly and I got to enjoy the wedding and feel good that everyone was safe and that we were doing everything we could to make our guests comfortable. Our guests especially appreciated us taking that extra step. I highly recommend Formation Health.

    Amazing Experience

    We were very happy we found Formation Heath to test at our January 2021 wedding. With most people not being vaccinated yet, it was very reassuring to know everyone has tested negative. Matt and his team were so accommodating from start to finish and even stayed late to test those that did not show up on time. They are very understanding and easy to work with. Could not have asked for a better experience. 10 out of 10 would recommend to anyone that needs testing at their wedding or any event.

    Amazing flexibility and top notch service

    We have used Formation Health for testing at two of our daughters' recent weddings. They could not have been more accommodating or professional. Details of the wedding changed up until the day of and they worked closely with us to be sure our guests had the best experience.
