Have you asked your officiant? Ours (a cantor) said that if the florist cannot provide one that his temple has a portable one that they can bring, if necessary. You would still probably want your florist to plan on decorating it, unless you or a friend/family member are feeling creative and want to just buy white fabric to wrap around it.

Party rental companies sometimes have them, too.

This was honestly our nightmare. Every time we found something promising, it didn't work out. In the end, we went with a very expensive rental from our florist. . . it was not exactly what we wanted, but it was beautiful and saved us a HUGE headache trying deal with yet another vendor, or trying to put something together ourselves.

Anyway. . . this was one of the companies we ran into. Our venue wasn't in their delivery area, but you seem a bit closer to them. I never got as far as pricing, but I think they have what you are looking for, so it would be worth contacting them to find out.

And there's also this one, which actually ships the chuppah to you for rental (and then you ship it back afterwards). We really liked these, but we didn't have anyone who could physically bring it to the venue and set it up for us-- if you do, then it could be a good option.

(PS-- some of them don't mention they are shippable on the summary page, but they are-- just click on the ones you like to double check the item page itself).

I was going to suggest party rental stores as well. Thats where we had to get ours since our florist doesnt provide them.

I just searched for local ones in my area and got some quotes. My thing was, I didnt want to have to set it up or take it down. Im lazy lol.
