Lost birth certificate

I've just begun the long process of changing my name (yuck), and realized my birth certificate is MIA. With both my parents and I moving several times in the past few years, it seems to have disappeared. Is it possible to change your last name without it?

    I think it depends on what state u live in. But your marriage certificate should be good.

    Not sure - but you should be able to order a certified copy of your original from the county that you were born in. Might be a good idea to order one just in case regardless.

    I have all our passports, social security cards, birth certificates and really important papers in a fire and water proof box/safe (1 hour) so I always know were they are when needed. Might be a great thing to add to a wedding registry for those without one.

    @D1: it's the social security replacement I'm having a hard time with. It says copies (even certified ones) and absolutely not accepted.

    Do you have a passport? Social Security will accept that in place of a birth certificate. http://www.socialsecurity.gov/online/ss-5.pdf

    I can't remember if I brought my birth certificate with me. I probably did as I always bring more documents than needed, but I only remember them specifically looking at my driver's license, marriage license, and old social security card.

    I just got back from SS office I had brought everything listed online & they only needed my driver lic & marriage certificate & the filled out form. I had Birth C on me & also my passport & my old SS card but they didnt need any! so call and ask or just head over and check. It shouldnt be a prob though (Im in NJ if that helps)

    My coworker lost his amongst his multiple moves and ordered a new one online.. it was legit apparently.

    In CA, you only need your DL & marriage license.

    (because you have to have a birth certificate to get a DL).

    For passport i think you only need to mail in the new form, old passport, new picture, fee! and maybe copy of marriage lic?

    I didn't need mine.. I just needed my ID and SSC.. After I got my SSC changed, I just needed my ID and marriage license for everything else.

    The marriage certificate alone should be fine. But the process varies from state to state. You can easily get a copy of your birth certificate and not even now, you might also need it later so better get it now. I also get a certified copy of my birth certificate from Vital Records Online when I lost it while moving to another house.
