It's intended to cover all eventualities. So it could be Mr., Ms., Mr. and Mrs., etc., depending on who is coming.

I had some people cross it out and write “Dr. “ but typically its for what you assumed

That’s where the guest writes the names of who is attending . The M is the first letter of the Mr or Mrs or Miss.
