Massachusetts name change

The intentions form definitely only covers surname. I believe if you wish to change your middle name in MA that is a separate process that would need to be done through the court. The only person whose answer you should follow for sure though is your town clerk.

Hi Heidi! I definitely think that you should contact your local government to check what the protocol is for changing your name in MA. I wouldn't want to give you the wrong information as this can vary from state to state!

I changed my name in Springfield (0/10 would not recommend...try a smaller social security office if you're in w. mass). I was able to change my middle name to include my confirmation name and I have three last names which they allowed as well.

Thanks! It looks like social security is only interested in first and last and a little flexible on middle. I wonder if I change there, if that document will let me change elsewhere including dmv in MA?
