Name change on Covid card

Through my work my husband and I got our vaccines (1-2) in Texas. It was a joint operation for teachers and their families to get it by local pharmacies and school nurses.

2 months after we got shots 1-2 done my husband changed his last to be his (step) dads.

Then we got married July 2021 and I took the his new last name.

1.5 months later we moved to AZ with 2 weeks notice for his new job.

Legally my name wasnt changed until Dec 2021

April 2022 We got our boosters but wanted our new names on the cards. Walgreens told us because they didn't do it we cant get it changed, we'd have to go back to TX to do that. BUT we don't have the direct pharmacy who did it in TX since it was a joint operation between local pharmacies and my school district.

I dont even know how to get it changed and everytime we travel it is a pain.

    Does Texas have the app connected to your state health department? You can log in with that and see if that works. Alternatively, I would maybe call someone from your old school district and see if they could help. They have to know which pharmacies were involved.

    Do you have a primary care doctor? I think they can provide you with a new card as long as they can verify the vaccines.
