Name Change on Passport

I put the city I got married in, as that was where the license was submitted and the subsequent certificate was issued from. I also put the date I got married.

I would do the city your marriage license is in and probably your wedding date.

I put the city we got married in and our wedding date.

I literally just did this a month ago. I put the city I changed my name as in SS because that is when I officially changed my name. It wasn't official at the courthouse.

I just did this a week ago. I went prepared with my marriage license and the person who helped me with it ask me if I had my drivers license with me in which I did and I had to show him it and he made a copy of it. I also had to put the city where I sent my marriage license to get registered and the dates that we got married along with my ssn.
