Name Change on Vaccine Cards?

I am currently in the middle of doing all the name change stuff, and I'm wondering what (if at all) i have to do for my covid vaccine card. Has anyone changed their name on their vaccine card? Will it matter if it has my maiden name on it?

    I think if you were to run into a problem with this it would look more sketchy if you were to alter your name on it (a legal document). I would just keep a copy of your marriage license on hand in case anyone gives you a hard time. It has your DOB on it so it’s hard to deny it would be you.

    I’m just carrying around both forms of photo ID even though one is expired. Unless you’re traveling it isn’t being looked at closely, unfortunately.

    Here's the advice I found if you were to lose your card...

    "If you didn't receive the vaccine card or lost it, your first step is to contact your vaccination provider, such as Rite Aid, CVS or a local government office. If you can't contact your provider, you can reach out to contact your state health department’s immunization information system. All vaccination providers must report COVID-19 vaccinations to their IIS.

    State health departments will either email or mail a copy of your vaccination card, although it may take one to two weeks."
    Every state is different so you might do a Google search for "lost vaccine card" and your state. You might try contacting them and asking to get a copy with your new name on it. I hadn't even considered this, it's a good question!

    Thanks! I actually saw this too, and I contacted them yesterday. My particular location said I had to request it via their vaccine portal, which I tried but there was no where to do so. It just gave me a PDF to download with my old information. Will probably have to try and call them again.

    I'm planning on changing it over with a booster. Hand in the card with my old name and have them make a new one with the new name. They will re-do the cards if there's an issue. They used white out on my friends card and I told her that's a big no-no for official documents and they re-did it on getting her second shot.

    Hi! After asking the initial question, I’ve found a few solutions that have worked for me:
    - 1) if I bring the physical card (my new booster card has my new name on it) I’ll also bring my marriage certificate with me. I haven’t had anyone question who I am or whether it is mine, but I bring it just in case. I assume this will be more difficult when traveling and things need to be more official.
    - 2) I contacted my local health department, and they were able to change my name in the system. From there, I was able to print a QR code certificate with all of my vaccine and booster information with my new name. This is also what people do when they’ve lost their vaccine cards. My local health department also set up a vaccine passport in the apple wallet which is also really helpful (and had my married name)
