Name Change Requirements

Hello! I am trying to send my information off to the social security administration and have a few questions that I can't seem to find online. Hopefully you can help!


1.) The form requests that you have a document stating your old name and new name. Our marriage certificate does not show an old and new name. It merely states that individual 1 (my name) and individual two (my husband's name) have wedded. Is this document enough to prove that a name change is requested?
2.) Do I sign the name change document as my old name or my new name?
Any advice would be greatly appreciated as phone lines are packed and phsyical offices are currently closed to the public.

    If you are taking your husband's name, then the marriage certificate shows both names (your maiden/current name and his legal name). I think it's phrased that way because people may change their name through court orders as well, which is where it would state that it was changed from A to B. I signed with my maiden name as that was my legal name at the time of the name change request.

    Hannah is correct the marriage certificate should be enough and you should sign with your current name, when you receive your new social security card you sign that with your new name. Make sure you practice signing your new name a few times. Sometimes muscle memory takes over and I sign my old name.

    In CA and more specifically LA I had to decide when we got our marriage license if I was going to change my name or not. There was a box at the bottom of the form to print my new name and that would be used for all the legal name changes. Also make sure you request at least 2 certified copies of your marriage certificate because some places will require an "original".

    I was recently married on March 8. Per the Social Security ministration you will need not only your marriage certificate, but either your passport or state issued ID or driver‘s license. You have to send originals when mailing. HTH.

    Has anyone figured out how to send in the supporting documents? Do you have to send your drivers license, original birth certificate and certified marriage license? What if you get pulled over?
