I am adding guests. For example, I add my friend, and then I add her boyfriend as a related person to her. Will he need to confirm if he will be attending my wedding? How would it look on my guests end? Thank you, in advance.

Usually just send an invite to your friend with a plus one. She then can respond that there will be the two of them -- Such and such name plus guest.

When they go to the RSVP list they can type in their name to confirm their attendance.
