My brother in law took my fiance on a trip to Miami last week with the other three groomsmen..... they went jetskiing and my cousin ended up in the emergency room with a broken ankle and will have to have surgery on it and on crutches for 20 weeks(we get married on april 30).... how do i have him sit or stand with us on the altar?

Put a chair on the alter or have him keep his crutches with him? Or ask him if he’d prefer to just sit in the front row.

Let him sit in the front row. Standing on crutches is miserable on top of any other pain. It sounds like you're focused on appearances rather than his health?

I agree with Sarah. Provide a him a chair at the alter. He can crutch down the aisle, sit during the ceremony next to the standing groomsmen, and crutch back down the aisle at the end. You could also ask him if he would feel more comfortable sitting on the front row instead of sitting at the alter.

Have a chair for him to sit & possibly another to prop his foot up.

Ankle breaks are miserable. It could have been so much worse! He may get a rolly thing though, I can’t remember the name - you bend your knee on it and can roll around. I’ve seen those used more recently. Better than the crutches I had when pregnant, I kept falling using those!
Either way, this stuff happens. Wedding and ankle breaks are part of life.

I thought the same thing ...
If the altar does not have to be reached by going up steps, give the man a chair to sit in and stool to place his foot on close by.
If the altar has steps, give the man a chair to sit in and a stool to place his foot on close by without having to go up any steps. Steps and crutches are not a good combination.

My friend's husband was on crutches for his own wedding. They had him sitting beside his bride for most of the ceremony, standing for some of the couple photos outside. They even made his cast match the wedding color theme. Not the end of the world to have one of the wedding party members on crutches!

This really isn't the end of the world. At least your fiance wasn't the one who got hurt! I think it's okay to feel disappointed or frustrated because inevitably this does change your vision slightly. But have your feelings then pick yourself up and figure out solutions. I'd give your photographer a heads up and maybe even ask if they have any ideas for funny poses to make the best of it. I'm sure your cousin would also prefer that his ankle was not broken but it will make for a good story someday. You cannot hold against him.
